Community Complex Care Nurse Care Coordinator HARP and Community Complex Care Programs 80 hours per fortnight with ADO hours (Shifts 0800-1630, 0830-1700 or 0900-1730) HARP (complex care) is looking to appoint a clinician to provide long service leave relief from 7th January to 29th March 2019 who is passionate about client centred care and assisting clients and their significant others to manage chronic disease in the community. The successful candidate will also have knowledge and experience in the areas of assessment and management of chronic disease, psychosocial concerns and cognition issues. Previous experience working in the community would be advantageous. This position is based in HARP, but the successful applicant may be required to work across other community complex care programs at times. Additionally, the successful candidate will have excellent communication and computer skills, with confidence in email and Microsoft office applications.
